Returning home after a skilled nursing stay can be overwhelming.
Our program creates an option for an additional stepping stone for patients between the receipt of skilled nursing rehabilitation services and their return to home.
Step to Home is ideal for patients:
- Needing additional assistance between the transition to home
- That can benefit from being monitored more closely
- Who will be receiving home health care services
- Needing more time to work on gaining independence
- Considering assisted living as a permanent residence
Program Insights:
- Patients must qualify for home care services. If no home care services are required, the respite stay program can be used instead.
- The Step to Home Program is a 2-week program, with the option to renew once for an additional 2-weeks.
- The maximum length of time of the Step to Home Program is 4 weeks.
- Patients using the Step to Home Program will qualify for the exclusive Step to Home daily rate.
- At the end of the program, patients may choose to become a resident, or return to their home in the community.
